For this assignment, students will construct a quality improvement plan utilizing the steps of the Plan-Do-Study-Act format.
The student will describe
the planning and implementation phase of the process.
The student will submit a paper in APA format, including the following elements:
1. Introduction to the problem
a. Students will identify a problem or area of practice that needs improvement. The student will clearly state the problem and give
examples of how this process has failed or may fail in the future.
b. Identify a source of data that will support your improvement. For example, the number of patients who are given an asthma action
plan compared to state or national data. If current data is not available in the organization, the student may include state or national
data that supports the area of improvement.
c. The student will articulate what they intend to accomplish with the change, what measures will be used to evaluate the change, and
how the change will result in improvement in the process.
2. Planning process
a. The student will predict what will happen with the change, personnel involved, if they need to develop a team to champion the change
and key people, financial implications, time needed to implement the change, any other resources needed, data to be collected,
where a pilot of the change can be implemented, if feasible, and for how long.
3. Implementation process
a. The student will not implement this process but will write out a timeline for accomplishing tasks, personnel needed, who is responsible
for each step, extra space if indicated, cost, expected findings, and barriers. Detail the type of data that will be collected during this
phase of the implementation and how it will be used. Include any areas of ethical concerns or protected information.
b. Describe how to assure the integrity of the data, how to implement the change on a larger scale, how and when to modify the plan,
and when to abandon the process.
4. Summary of the Plan
a. The student will describe the lessons learned through this assignment, problems encountered, main points from the paper, how data
can strengthen a quality improvement project, and the next steps if the implementation is planned.
